Welcome! A prospective Wave players is a player new to Wave. You have not played on a Wave team within the last 2 seasons.

1) COMPLETE THE "WAVE CONTACT INFO / MEDICAL WAIVER" below. YOU MUST BE A US LACROSSE member to participate.

            - If you are not a member, go to www.uslacrosse.org and easily register. You will instantaneously get a membership #

              and be able to enter that on the form below.

             -If your membership is expired, go to www.uslacrosse.org to renew. Then fill in the new expiration date.
